Why do you need an example of your exact document? Cite it the way it says to cite court filings.

This is for a worksheet we have in preparation of our final LRW assignment. We have been given a worksheet and asked how to cite certain documents, but I can't figure out how to cite this document. We are allowed to collaborate with students in the class, however, my group can't figure out how to do it.
Any help would be appreciated.
