LSAC agreed that I'm broke, so the Y and H waivers came from that. S has a whole separate form that you must fill out, regardless of if you received the LSAC waiver. IIRC tax information from the last year and some proof of US/Canadian residency. I'm sure Y and H will tell you that they leave it to LSAC for need-based waivers, i.e. if you qualify for it your app fee will be waived automatically on the application.
