I would not mention the second reason. I think the jeopardize reason is okay. Maybe also put in a line saying something like you thought another academic LOR was more indicative of your abilities because you had the opportunity to work longer with them/more closely with them? Checking in, 171/Above Average (International), mylsn says it's a coin flip but LSAC says chances are around 30-40%, I guess it's still well worth a shot

During the information session that I attended at Columbia, there was a girl that asked this same question about not being able to ask for a recommendation due to the jeopardizing reason. Jill Nussbaum said that it was completely fine as long as you explain it in an addendum.

Is it OK to have a colleague who was basically at the same position/level as you write your professional LOR?
I know they'd do a better job than my former supervisor, who was pretty MIA/literally spelled her own name wrong on her business card.
