OP here-
my license was only suspended once.
The background check says violation- failure to appear with two dates (one of the dates is the traffic stop and the other is what I guess is the date they didn't receive payment) and then a suspension date.
The suspension was a result of forgetting to keep paying the fine and my license was reinstated in full shortly after the suspension date once I went to the DMV and payed the outstanding fines.

For what its worth, I know this is something I would have had to disclose on the bar, but the background check only asked for convictions and didn't ask about traffic violations. It was a pretty simple questionaire asking for identify stuff, education, and convictions. They asked for my license number, etc. but I figured this was more to verify my identify and pull up my record for anything major/criminal like a DUI.

what kind of ttt firm cares about this shit even a little bit. this profession is trashed. used to be we could get duis and someone from teh firm would come help you out of it. now they give new hires shit for traffic tickets.
