Formatting a DS/PS - heading, name, etc.

I want to get some opinions on PS/DS formatting. I just have my name and LSAC number in the header to the right and then "Personal Statement" centered just above the text. Is that fine? I'm assuming I don't need page numbers since its only 2 pages.
Should I do the same for my DS? I've noticed some schools do not call them "Diversity Statements" but rather say something like "optional statement." Should I use whatever they call it as my heading?
I searched the site and couldn't find much about the DS heading question. Any help is appreciated.
