PS Document Format?
This question may seem dumb, but I can't find the answer anywhere. Is there a specific way to format the PS document? Like, should my name be a header? Should it be on its own line? Should I include my LSAC number on the top? top of every page? Should I include page numbers? Anything else like that?

I don't think there's a standard format you have to follow for most schools, but including name, title (like "Personal Statement", not something flowery), and LSAC # equally spaced apart in a header is a common and sensible approach. This method doesn't take up space on the page (thereby adding unnecessary length to the document) while ensuring identification info appears on every page.

It doesn't really matter, since you're uploading the thing to each app anyway, so it's packaged with the rest of your application.

Do you know if it’s supposed to be PDF or word format?

I would do a pdf, just because I've had issues with documents looking weird between different versions of Word.
And the advice to put "Name - LSAC nr" on one line and "Personal Statement" (or "Diversity statement") on the other is spot on.
