Single spaced or double spaced?
I know that 2 pages is the general requirement, but is that single or double spaced?

Double spaced is the general norm AFAIK. I went with the rough formatting below...
Margins: Normal (1"x1"x1"x1")
Font: 11 pt Times New Roman
Line Spacing: Double
Paragraph Alignment: Justified
New Paragraph: 1/2" indent
Header:
Left <-> Right
My Name <-> Attachment Desc (pg. #/#)
LSAC ACT #: L########## <-> n/a
Footer:
None
I think the above gives you a nice and clean looking document for your statement. I'd definitely include a header or some form with your basic identifying information and what the file actually is.
Margins: Normal (1"x1"x1"x1")
Font: 11 pt Times New Roman
Line Spacing: Double
Paragraph Alignment: Justified
New Paragraph: 1/2" indent

Left <-> Right
