Title

Stupid question I'm sure, but if I come up with a title for my personal statement it pushed my statement onto the forbidden third page. Should I just label the attachment Personal statement sans title or come up with something witty and slip onto the third page?

Don't go onto the third page. Keep in mind a title is probably not necessary, though if you're dead-set on your awesome title you could perhaps put it as a header?

I agree with the above poster. Headers don't cut into your allotted space.

A header with my name and LSAC # will definitely show up in the attachment right?

Yes. Plus if you're worried, you can always review the PDF before you submit to make sure it's there.

Good call. There are a few pages in between certain attachments that say "this page is blank" do I have an extra page accidentally or something?

Some schools set it up that way, I suppose to make it easier to tell where one attachment ends and another begins. Don't worry :)
