So, I have a few things I need to disclose for this upcoming cycle and am not sure the best way to go about it. I have two criminal charges but no convictions, one juvenile from 2003 and the other an underage possession from 2007. On top of that, I think I have one alcohol writeup from freshman year of ug (2006) that I think I need to disclose as well, but haven't had any problems since then.

My question for you guys is, is it acceptable to put all of them together in one addendum and then right a paragraph about what I've taken away from them as a whole, or should I write a separate addendum addressing each incident?

I'm in the same situation.
My plan is to write one for my criminal record, another for my university infractions, and another for my other thing.
I figure having one for each box you checked will be best because all schools might not all ask for all of the information, and it will hopefully be easier to read.
